Conservative Treatment of Transmural Perforation with Mediastinal Leakage after Balloon Dilation: A Case Report
نویسندگان
چکیده
loon dilation is 0-14% (1-3). An esophageal perforation is classified into three types: a type I-intramural perforation; a type II-transmural perforation; and a type IIItransmural perforation with mediastinal leakage (4). Fasting, parenteral alimentation, and antibiotics are generally used to treat types-I and II esophageal perforations. However, a type III esophageal perforation showing progressive clinical deterioration needs to be treated surgically (4, 5). There are no reports of a successful conservative treatment for a type III esophageal rupture after esophageal balloon dilation. We report a case of a successful conservative, non-surgical treatment for a transmural perforation with mediastinal leakage after a balloon dilation.

An 80-year-oldmanwas admitted for cholestatic jaundice. He had undergone Billroth II gastrectomy 32 years earlier due to an ulcer. Magnetic resonance cholangiopancreatography revealed choledocholithiasis. At endoscopic retrograde cholangiopancreatography (ERCP), reverse sphincterotomy, balloon dilation of the sphincterotomy, and bile duct stone removal were performed (●" Fig.1 and ●" Fig.2). Sh...

A 56-year old woman was transferred to our hospital 1 day after balloon dilation for recently diagnosed achalasia that had been complicated by transmural esophageal perforation. Physical examination revealed tachycardia and fever, as well as left-sided pleural fluid. Emergency surgery was performed, which involved transhiatal primary closure of a 4-cm long transmural perforation, debridement of...
متن کاملComplete endoscopic closure (clipping) of a large esophageal perforation after pneumatic dilation in a patient with achalasia.
The risk of esophageal perforation following endoscopic balloon dilation for achalasia is in the range of 1%-5%, with a mortality rate of 1%-20%. Perforations need to be recognized early, and, if reasonable, an immediate endoscopic repair should be pursued quickly. Herein, we report a case of successful endoscopic closure by clipping of a large iatrogenic perforation in a patient with achalasia...

Introduction: Although perforation of the esophagus, in the anterior cervical spine fixation, is well established, cases with delayed onset, especially cases that present pseudodiverticulum, are not common. In addition, management of the perforation in this situation is debated. Case Report: Delayed esophageal pseudodiverticulum was managed in two patients with a history of anterior spine fi...
